Primary Care Occupational Therapy

Our Primary Care Occupational Therapy (PCOT) pathway combines digital support and therapy to help you manage your health and maintain your independence.

What will I need?

To use the Connect Me PCOT service, you will need:

  • A smartphone or tablet with the My Inhealthcare app installed.
  • An email address or mobile number.
  • Your Community Health Index (CHI) number. You will find this is on your referral letter.

How to use the Connect Me PCOT pathway

You’ll be referred to the PCOT service by your GP, via self-referral, or by contacting the service directly. A healthcare professional will enrol you on the Connect Me platform and activate the PCOT service for you.

Once enrolled, you’ll receive an email or text with instructions how to access the My Inhealthcare app. Remember to turn on notifications in the app settings so you receive questionnaires and messages from your clinician.

What does the service do?

When you first join the service, you will be sent an initial screening questionnaire. This allows clinicians to assess your needs and plan your care pathway. If needed, the service can also send pre-appointment and ongoing care questionnaires to monitor progress and support therapy.

The occupational therapist assigned to you will have access to all the information you provide in the questionnaires. You will also receive targeted information and resources to help you manage your condition.

The PCOT service also provides secure two-way messaging between you and your clinician.

What happens with my results?

When you submit your questionnaire, the occupational therapy team will review your responses. Once reviewed, they can decide if you need an appointment or referral, tailor your therapy plan, and track your progress.

Your responses and clinician notes form part of your clinical record. This is accessible via the Inhealthcare platform.

What else should I know?

You can opt out of the service at any time by selecting the “Opt Out” option in the app.
