Primary Care Occupational Therapy (PCOT)

PCOT combines digital support and therapy to help you maintain independence and manage your health.

What will I need?

To use the NHS Connect Me PCOT Service, you will need:

  • A smartphone or tablet with the My Inhealthcare app
  • An email address or mobile number
  • Your CHI number (found on your referral letter)

How to use the NHS Connect Me PCOT Pathway

  • You’ll be referred to the PCOT service by your GP, via self-referral, or by contacting the service directly.
  • A healthcare professional will enrol you on the Connect Me platform and activate the PCOT service for you.
  • You’ll receive an email or text with instructions to access the My Inhealthcare app.
  • Turn on notifications so you receive questionnaires and messages from your clinician.

What does the service do?

  • Sends you an initial screening questionnaire to assess your needs and guide your care pathway.
  • If needed, sends pre-appointment and ongoing questionnaires to monitor progress and support therapy.
  • Provides targeted information and resources to help you self-manage your condition.
  • Enables your Occupational Therapist to view your answers and personalise your care.
  • Allows two-way secure messaging between you and your clinician.

What happens with my results?

Your occupational therapy team reviews your questionnaire responses to:

  • Decide if you need an appointment or referral
  • Tailor your therapy plan
  • Track your progress

Your responses and clinician notes form part of your clinical record. You can access these through the Inhealthcare platform.

What else should I know?

You can opt out at any time via the “Opt Out” option in the app.
